Scanned image color fades after preview finishes

I do a lot of 35mm kodachrome batch scans. The images are mostly faces. Occasionally one or two slides in the batch will scan as an almost black & white image. If I do a preview scan, the preview image starts out in color but loses almost all color once the preview finishes.
I am using a Nikon coolscan ls4000 and the Silverfast stand-alone interface. I am using IT-8 calibration. Digital Ice is enabled.
I get the same results with Photoshop CS plugin. I just installed the latest silverfast AI release yesterday but the results are the same. Scanning with the ektachrome, fujichrome or no calibration gives the same result.
Could there be some auto or other option which I have improperly set up? Scanning the same slide with the new Nikon Scan shows me that post-processing could be the cause although I have not yet checked all the Nikon scan options to see which one could be causing the color change.
