On startup you should see a dialog, where you can enter your name and serial number.

You can use your version of SilverFast for any OS that is supported by SilverFast, like OS9, OSX, Win.
To your question: no you do not have to pay extra, if you use OSX 10.4.

SilverFast HDR can read images from files instead of scanning them. HDR can be usefull, if you want to postprocess your images at a later time
