Problems scanning 6X6 B&W negative redux

Attached is the result of trying to do a 16BIT grayscale HDR scan, a 16 bit grayscale scan I have also included a screenshot of the setup page.
I used the same frame settings but with the software set to 48 bit color and got good results. What could be causing this? Have I got something set up incorrectly?
