Problem scanning B&W negs with 9000 qnd glass holder

Here is a screenshot of the preview I am getting and then another of the finished scan. As you can see on the preview I had negafix set for Kodak tmax 400.. I had ME turned on... I never expected results as bad as this. What could be happening?
